UPDATE: Nope, it isn't a new NR  In the 2nd heat Zofkova set a new NR with a 0.02 less than Scalia

Apologies - The Australian women's 4 x 200 were disqualified in 2001 when the 3 other swimmers jumped into the pool before the last team had finished.

Not because their last swimmer left the pool before the race ended,
