No, it's not a metaphor, he was literally diagnosed with a heart condition at the last medical check up. As you know the Italian NOC is strict compared to other countries about this and athletes don't get the authorization to compete at elite levels with such problems.

I'm sorry but this is going out of hand. If you have two athletes as strong as the greeks, is yours federations blame only for not being able to rise a third athlete with a decent level enough to figth for the team quota.

Yes but Italy is guaranteed to win the quota by ranking in case someone else gets the European quota. So, one way or another, Italy will have 2 men and therefore 2 teams.
