Rafa Nadal is unqualified due to missing minimum  Davis Cup representation level?

He can argue that he didn't reach it due to an injury or to a high internal-competition of the country.

I'd say the first option :p

The second option might be possible if you're the world number 40 or something, but not if you're Nadal :d

Yep. I think the second opcion is usefull to players like Pablo Carreño or Albert Ramos-Viñolas, who don't reach the minimum either.

Tennis Canada told Radio Canada that Raonic, Pospisil and Bouchard will play singles in Rio, so no surprise there. No word from them on doubles yet.

Nestor must be one of the candidates for doubles, as well, right?

Well he's not in the top 10 so he has to compete with other combined ranked teams to qualify. Of course Nestor + Pospisil have a combined rank of 33 which means they should easily make it in.

Almost certainly, I just don't think double places have been confirmed yet.

Raonic and Pospisil have announced they are competing in doubles together. So looks like Nestor is out.

That was back in 2015, but you're probably right. I've heard Nestor is hard to partner with, but I don't expect Raonic to look good once they face a true doubles team.

Top 10 players can choose whoever they want to play with.
